The Straight Clinicae Template has three different functions:

1.- To center single implants, either 4 or 5 mm. diameter.
2.- To separate implants with a diameter of 4 or 5 mm at a distance of 3mm.
3.- To parallelize mandible surgical socket preparations.

SCT | Single

Centering Single Implants

 

(1)The smaller corresponds to a 4 mm. diameter implant and has a central hole of 2mm. which allows the initial drill, to mark the central place. The margin has incorporated the security distance of 1,5mm for the adjacent teeth. If the circle fits in the space between those teeth, we know that our implant of diameter 4mm has 1.5mm to each side.

(2) The other extreme corresponds to a 5mm. diameter implant and has the same characteristics described for the previous point. We can choose the using one diameter or the other one, 4 or 5, depending of the space between the adjacent teeth.

Both guides are fabricated with the right precision to guarantee the distance between implants. You just must use the pilot drill 2mm into the hole and the implants will be positioned to the exactly secure distance and parallel. .

Separate Implants at the security distance

 

When several implants are placed, or simply we want to place an implant besides another, we will use the lateral top for doing the initial perforation of the socket. Note that the Straight Clinicae template has two lateral tops, one near the small point, of 4mm. and other in the big side, for implants with diameter 5mm.

CPlacing the top against the edge of the proximal implant we use the opposite hole to the top, beginning the sequence of drilling which ends by placing an implant with 4 mm diameter at a distance of 3mm to the edge of the implant taken as reference, independently of its diameter. If we use the other extreme, we will place at a distance of 3mm an implant of 5mm. diameter. We use the transversals tops 1 and 2 for doing surgical preparations, taking as reference an adjacent implant.

SCT | Parallelize

Parallelize Mandible Implants

 

The Straight Clinicae Template can be used to ensure that the placement of mandible implants are parallel. The placement of perpendicular implants to the mandible is easier at a variable distances at the operator will. The anatomic characteristic of the mandible allows place perpendicular implants to the oclusal plane. For facility the maneuvers of taking impressions, the implants must be separated and orientated spatially, in a way that the parallelism between them is predominant. Colliding implant trajectories, complicate the impressions, and obligate to use angulate abutments, of higher price.

Procedure to parallelize:

a) With a Lindemman burr prepare the crestal edge of the mandible until having an adequate and homogeneous thickness. This edge should be parallel to the bipupilar plane.

b) Identify the emergence of the mental nerve and then establish a safety margin to prepare the most distal socket implant position on the left or right, whichever you prefer.

c) Place a check pin orientation and verify front, profile and occlusal view in order to obtain a preparation that will be perpendicular to the plane created on the a) step.

d) Insert the first implant. This will be the guide to the orientation of the others.
e) Place an abutment screw on the implant an then insert the straight template so it is snug but can turn loose pivoting on the implant

f) As the mandible could have had recent extractions, bone defects, etc. We will use the best places in a free way. By moving the guide, we will find an orifice that fits the site that we establish. When drilling through it, its thickness 2 mm, indicates a parallel to the implant guide which is at the point of the straight guide.

g) We proceed to make another socket by drilling freely and moving the guide and calculating that there is adequate separation between the perforations.

h) All sockets can be made parallel to the first implant or at any time and for convenience you can set the guide with each screw with other implants already placed, which in turn is parallel to the first of the series, and continue drilling. If we place a guide pin in each hole we will check the parallelism of the holes drilled, as well as the distance that separates them.
